I understand it is my responsibility as the parent to provide STEM Summer Camp Staff with an Epi Pen Daily while my child is attending the camp. It is the parent’s daily responsibility to personally hand the EPI Pen directly to STEM Summer Camp staff for safe storage daily, to ensure the safety for all children OR to provide an Epi Pen that will remain on location with STEM Summer Camp Staff for the duration of the summer.

(An Emergency Contact is a 3rd person who can be contacted only in case of an emergency when either parent listed above cannot be reached.)
I give permission for the STEM Camp Staff to contact the emergency contact and release my child into their custody ONLY in case of an emergency where myself or the other parent listed above cannot be reached. I understand that it is my responsibility to inform the STEM Summer Camp staff if someone other than the parents listed on this registration form will be picking up my child. I understand that the STEM Summer Camp Staff are required to check photo ID for any other individuals who I authorize to pick my child up and they will ONLY release my child to that individual if I have given the full name and instruction to release my child to that individual (i.e. Grandma, Grandpa, Auntie, etc.)
